CRM for Med Spas: From Ad Lead to Rebooked Client in HighLevel

A CRM for med spas handles everything before and after the treatment room. It answers ad leads in minutes, books consults with a deposit, sends the treatment plan and rebooks each client when their next treatment is due. HighLevel does this with workflows, calendars, payments and reviews. However, it is not an EMR, so you keep charting and clinical photos in a tool like Aesthetic Record, Pabau or Zenoti.

It is Thursday at 8:40 p.m. when Ashley taps a lip filler ad on Instagram and fills in the form. The med spa answers the next morning at 11, after the injector’s first two patients. By then, Ashley has booked a consult at the clinic that texted her back first. A good CRM for med spas stops that leak. It replies in a minute, offers consult times and takes a small deposit, so the slot is real.

The quieter leak comes later. A client gets Botox in March, loves the result and never hears from you again. So in July she books wherever an ad catches her. In short, med spa revenue lives in the second, third and fourth visit, and those depend on timing.

This guide shows how to build that system in HighLevel: the client pipeline, three automations, deposits, rebooking by treatment, reviews and memberships. It also covers HIPAA, before-and-after photo rules and what stays in your EMR. It is part of our GoHighLevel industry guides.

What a med spa needs from a CRM (and why you still need an EMR)

An EMR or aesthetic practice platform runs the treatment: charting, consents, injectable lots and clinical photos. A CRM for med spas runs the relationship around it. That means the ad lead, the consult, the plan, the reminder and the rebooking. So think of HighLevel as your front office and marketing system, connected to the clinical tool you already trust.

Common med spa problems and how HighLevel solves them

List the leaks first. A CRM for med spas maps each one to a feature, so you can fix them in order of cost.

Med spa problem What HighLevel does Feature
Ad leads go cold overnight Calls or texts each lead within minutes and offers consult times Workflows + AI
Free consults that no-show Takes a deposit at booking and sends 24 h and 2 h reminders Paid calendars
Plans that never get accepted Sends the plan with a text-to-pay link and a coordinator task Estimates + payments
Clients forget their next treatment Texts a booking link before the due date the provider set Custom date field + workflow
One-off visits, no loyalty Offers a membership after the second treatment Recurring products
Few reviews Sends a review request the day after each treatment Reputation management
DMs scattered across apps Puts Instagram, Facebook, SMS and WhatsApp in one thread Conversations inbox
No view of ad return Shows cost per consult, show rate and plan acceptance Reporting + Ad Manager

CRM vs EMR: the honest med spa software stack

An industry guide by LeadSquared puts it simply: the EMR manages what happens during treatment, while the CRM manages everything before and after the visit. That split is useful because each tool is strong on one side. For example, Aesthetic Record lists a complete EMR, chart sign-off and before-and-after photo management. HighLevel, in turn, is built for funnels, follow-up and messaging.

So the usual setup has two layers. Charting, consents, clinical photos and injectable inventory stay in the EMR. Meanwhile, leads, consult booking, reminders, rebooking, reviews and memberships run in HighLevel. The two connect through webhooks and the API, or a connector such as Zapier. Then decide which system owns the appointment book, because two live calendars cause double bookings.

A client pipeline in your CRM for med spas

A med spa pipeline follows the client from first click to regular, not just to the first sale. Workflows move the card when a lead books, pays a deposit, shows up, accepts a plan or comes due again. As a result, the owner sees where ad money turns into revenue and where it leaks.

Stage What moves the client here
New lead Lead ad, offer funnel, DM or missed call
Consult booked Booked on the paid consult calendar, deposit received
Consult done Appointment marked Showed
Plan sent Estimate or treatment plan sent with a payment link
Treated Treatment appointment marked Showed; due date saved
Rebook due Custom date reminder fires before the next treatment is due

Deposits, treatment plans and memberships

Deposits are the simplest fix for consult no-shows in any CRM for med spas. In HighLevel, a calendar can collect payment at booking through a connected processor such as Stripe. Write the policy in plain words on the booking page: the amount, when it is credited to treatment and when it is lost. That way, nobody feels surprised.

After the consult, send the plan as an estimate or invoice with a text-to-pay link while the client is still excited. For loyal clients, a monthly membership built with recurring products smooths revenue between injection cycles. You can also sell packages for laser series, then track the remaining sessions with a custom field. Online education, such as skincare routines, fits in courses and communities.

Three automations every CRM for med spas should run

1. Ad lead → consult booked in minutes

The trigger is Facebook Lead Form Submitted, or your website form. First, HighLevel creates the opportunity in New lead with a source tag. Then a call connect rings the front desk and dials the lead while interest is high. If nobody answers, a text with the consult booking link goes out at once, and an email with the offer follows ten minutes later.

After that, short nudges go out on day 1, 3 and 7, and every one of them stops as soon as the lead replies or books. After hours, Conversation AI can answer “how much is lip filler?” and offer times. That way, your CRM for med spas works the night shift without sounding like a robot.

2. Paid consult → treatment plan accepted

This workflow starts when someone books the paid consult calendar. It confirms the time, repeats the deposit policy and sends the intake and consent links from your EMR or HighLevel forms. Reminders follow 24 hours and 2 hours before. If the client no-shows, she gets a friendly reschedule link and the front desk gets a task.

If she shows, the workflow waits an hour and sends the treatment plan with a text-to-pay link. Then, if nothing is accepted in three days, a coordinator gets a task to call. In short, the plan never sits in an inbox while the client cools off.

3. Rebook at the right interval, then a membership

Every treatment has its own rhythm, and the provider, not the CRM, sets it. So after each visit, the provider or front desk saves a “next treatment due” date on the contact. Your EMR can also send it by webhook. Then the Custom Date Reminder trigger fires ten days before that date and texts a booking link.

Treatment What the CRM stores How the reminder works
Neurotoxin (e.g. Botox) Last treatment date + due date set by the provider Text 10 days before due, email 5 days later
Dermal filler Area treated + follow-up and touch-up dates Follow-up check first, then a longer rebook reminder
Laser series Sessions bought, sessions used, next session date Reminder per session until the package ends
Facials and peels Membership status Monthly booking nudge for members

If the client hasn’t booked five days after the reminder, an email offers your membership. Separately, a short workflow sends a review request the day after each treatment. Keep these messages about the client’s own care, not discounts, because that is both kinder and safer under HIPAA.

Instagram DMs, texting and AI replies for med spas

Most aesthetic leads arrive as DMs, not phone calls. So in a CRM for med spas, the inbox matters as much as the calendar. HighLevel puts Instagram and Facebook DMs, SMS, email, web chat and WhatsApp in one thread per client. The team replies from the mobile app between appointments, and snippets keep prices and prep notes consistent.

In the US, register A2P 10DLC before you text, because HighLevel requires it for local numbers (A2P guide). Then add Conversation AI for questions that repeat all day, such as price ranges, downtime and parking. However, keep medical questions with a licensed provider. If HIPAA is enabled, also check which AI features your setup covers before AI reads treatment details.

What HighLevel’s official Medical Spa snapshot includes

HighLevel publishes a free Medical Spa snapshot playbook. It is a useful starting kit, but it is a generic marketing template with the industry name swapped in. It names no treatments, so the med spa logic is yours to build. Out of the box, it includes:

  • A “Medical Spa” website with a Home page (services and FAQ) and a Link in Bio page.
  • A “Medical Spa Offer” funnel: an opt-in step, then an appointment booking page.
  • The “Marketing Form – Claim Offer” and a “Marketing Pipeline” whose stages you define.
  • Two named workflows: “New Lead Nurture (Fast 5) – Claim Offer”, with email, SMS and a voicemail drop, and “Long Term Nurture”.
  • Service calendars tagged “Medical Spa Services”, a simple calendar and an event calendar for consultations.
  • Custom values, a chat widget, missed call text back, one ad template and a Voice AI prompt.

You still build the pipeline stages, the paid consult calendar, treatment calendars per provider and room, the plan follow-up and the rebook dates. Memberships, review timing and every message are also yours, since the guide ships no copy. Load the kit through snapshots when you create the sub-account.

How to set up HighLevel as a CRM for med spas in 8 steps

  1. Decide on HIPAA first. If you will store treatment data, enable the HIPAA add-on and sign the BAA before importing anything, because it can’t be removed later.
  2. Create the sub-account. Load the Medical Spa snapshot or start blank, then set the business profile, hours and custom values.
  3. Import clients. Import a CSV with last treatment, treatment type, next due date, membership status and marketing consent.
  4. Build the pipelines. A lead-to-consult pipeline and a treatment-to-rebook pipeline in the CRM.
  5. Set up calendars. A paid consult calendar with a deposit, then treatment calendars per provider and room with buffers.
  6. Connect phone and social. Buy a number, register A2P 10DLC, turn on missed call text back, and connect Instagram, Facebook and Google.
  7. Connect payments and offers. Connect Stripe, create packages and a membership product, and publish the offer funnel.
  8. Publish workflows and reporting. Test the three automations end to end, then track cost per consult, show rate and rebook rate in Reporting.
Eight step setup guide for a CRM for med spas in HighLevel

What a CRM for med spas costs with HighLevel

HighLevel charges a flat plan plus usage. A single-location med spa usually starts on Starter at $97 a month, with unlimited contacts and users. Usage comes on top: $1.15 a month for a US number and $0.00747 per SMS segment plus carrier fees. A2P registration costs $22.50 one-time plus a monthly campaign fee.

The big line is HIPAA. The add-on costs $297 a month, it is bought at the agency level and it can’t be cancelled once enabled. So a med spa texting about 3,000 segments a month pays roughly $125–$170 without HIPAA and about $420–$470 with it. An AI plan at $50 or $97 a month comes on top. These numbers are illustrative. See our pricing guide and the AI Employee pricing.

Honest limits and alternatives

HighLevel is a CRM for med spas, not a clinical system. So check these gaps before you move anything.

  • No EMR or charting. Treatment notes, injection maps and sign-offs belong in a clinical tool.
  • No injectable lot tracking. Lot and expiry records stay in your EMR or inventory system.
  • No photo comparison tool. And HighLevel’s ideas board lists “Exposed media uploads” as under review: media links don’t need a login, even with HIPAA on (ideas board).
  • HIPAA is off by default. It is a $297 a month add-on with a BAA signed under Settings › Compliance, enabled per sub-account and permanent (HighLevel help).
  • No full POS. You get invoices, text-to-pay and Tap to Pay, but no retail stock control.

Here is a neutral comparison with clinical tools people pair with a CRM. The cells come from Aesthetic Record’s site, Pabau’s software guide and the LeadSquared guide. They are vendor pages, and “Check vendor” means they didn’t say.

HIPAA, texting and marketing: whether a cash-pay med spa is a HIPAA covered entity is a legal question, so many treat treatment data as PHI anyway. HHS says PHI can’t be used for marketing without written authorization, with limited exceptions such as treatment communications (HHS). Marketing texts also need prior consent under the TCPA (FCC). Medical director and good-faith exam rules vary by state. This is not legal advice.

Before-and-after photos: consent, storage and “typical results”

This is the part most guides on CRM for med spas skip. Before-and-after photos sell treatments, but they carry three separate duties. First, consent: if HIPAA applies, a photo used in an ad is PHI used for marketing, so you need the client’s signed authorization. Second, storage: keep clinical photos in your EMR, not in HighLevel’s media library, because of the open media issue above.

Third, honesty. The FTC’s Endorsement Guides, revised in 2023, say that if you can’t show an endorser’s result is what people generally get, the ad must make the generally expected results clear. So pick representative cases, don’t edit the results and add a plain line such as “results vary”. Then log each authorization in a custom field, so your social team knows which photos are cleared.

Frequently asked questions

What is the difference between a med spa CRM and an EMR?

An EMR documents the treatment: charting, consents, injection maps, lots and clinical photos. A CRM manages the relationship before and after the visit: ad leads, consult booking, reminders, treatment plans, rebooking, reviews and memberships. Most med spas need both. HighLevel is a strong CRM for med spas, while tools such as Aesthetic Record or Pabau cover the clinical side.

Is HighLevel HIPAA compliant for med spas?

Not by default. HighLevel offers a HIPAA add-on at $297 a month, bought at the agency level, with a BAA signed under Settings › Compliance. You then enable it per sub-account, and it can’t be removed once on. Even with HIPAA enabled, the ideas board lists exposed media links as under review, so keep clinical photos in your EMR.

Can I take deposits when clients book a consult?

Yes. A HighLevel calendar can collect payment at booking through a connected processor such as Stripe. Put the deposit amount and policy on the booking page and in the confirmation text, including when it is credited to treatment and when it is forfeited. Deposits filter out casual bookings and protect injector time, especially for consults that come from ads.

How do I remind clients when their next Botox treatment is due?

Store a “next treatment due” date on each contact after the visit, set by the provider or synced from your EMR. Then a workflow with the Custom Date Reminder trigger texts a booking link ten days before that date and emails a reminder later. The provider sets the interval for each client, so the CRM only handles timing and booking.

How do I sell memberships and packages?

Create a recurring product for the membership and one-time products for packages, then connect Stripe. Offer the membership after a client’s second treatment, when the value is clear. For laser or facial packages, track sessions bought and used in custom fields, so reminders continue until the package ends. Keep the terms simple and the cancellation process clear.

How much does a med spa CRM cost?

HighLevel Starter costs $97 a month with unlimited contacts and users. Usage adds about $1.15 a month for a number and $0.00747 per SMS segment plus carrier fees. If you need HIPAA, the add-on is $297 a month. So a single-location med spa pays roughly $125–$170 a month without HIPAA, or about $420–$470 with it.

How fast should a med spa respond to ad leads?

Within minutes, ideally while the client still has the ad open. Leads that wait until the next day have often booked elsewhere. In HighLevel, a workflow can ring your front desk and connect the lead right away, then send a text with consult times if nobody answers. After hours, Conversation AI or Voice AI can answer and book.
