GoHighLevel forms, surveys and quizzes: builder, logic, payments and automation

Quick answer

GoHighLevel includes three builders on every plan: forms to capture leads and orders (with a payment element), surveys with conditional logic to qualify leads, and quizzes with scoring, categories and result pages. Everything maps to CRM fields, embeds on any site and triggers workflows when submitted, so there is no need for Typeform, Jotform or a separate quiz tool.

Based on HighLevel’s forms, surveys and quiz builder articles, updated April 30 and October 6, 2026.

A med spa asked every website visitor the same thing: name, email, phone, “how can we help?”. Leads came in, but the team spent hours calling people who only wanted prices. They replaced the form with a four-question quiz: treatment, budget, timeline, past treatments. Same traffic, fewer leads, twice the bookings.

The GoHighLevel form builder is where that kind of change happens. This page explains when to use a form, a survey or a quiz, the elements and logic available, how payments and embeds work and how to connect every submission to an automatic follow-up.

Quizzes with scoring

Quizzes are the most underused builder in HighLevel. You give points to each answer, group them into categories (for example beginner, intermediate, advanced) and show a different result page and call to action for each score tier.

  1. Define the outcomes. Three or four categories or tiers that lead to different offers.
  2. Write five to eight questions. Each answer adds points to a category.
  3. Design the results page. Header, score-tier text, colors and a call to action per tier.
  4. Map to the CRM. Save overall score, tier and category scores to contact fields.
  5. Automate the follow-up. Use the Quiz Submitted trigger and branch by tier: book a call, send a guide or nurture.

Forms that take payments

Order-style forms can include a payment element connected to your products and payment gateway, so a deposit, consultation fee or product sale happens in the same step as the lead capture. Funnels and websites.

After the submission: automations

  1. Instant reply. SMS, email or WhatsApp within a minute of the submission.
  2. Create the opportunity. Add the lead to the right pipeline stage. CRM and pipelines.
  3. Assign and notify. Round-robin the lead to a team member and alert them.
  4. Book the call. Send a calendar link or let Conversation AI book it.
  5. Nurture. Different sequences depending on the answers.

Frequently asked questions

Does GoHighLevel have a form builder?

Yes. Every plan includes forms, surveys and quizzes. They are built in the Sites area, embedded in HighLevel funnels and websites or on any other site, and every submission creates or updates a contact in the CRM.

What is the difference between forms, surveys and quizzes?

Forms capture information on one screen, such as a contact or booking request. Surveys ask several questions with conditional logic to qualify a lead. Quizzes add scoring, categories and a results page based on the score.

Can GoHighLevel forms take payments?

Yes. Use an order-style template, add the payment element, connect a supported payment gateway and choose your products. Manual methods such as cash on delivery can also appear if enabled.

Can I embed GoHighLevel forms on WordPress or another site?

Yes. Forms, surveys and quizzes can be embedded with code on any website or shared as a direct link.

Can a quiz send different follow-ups based on the score?

Yes. Quiz scores, tiers and categories are saved to contact fields, and the Quiz Submitted workflow trigger lets you branch the follow-up by score or category.

Can I convert a form into a survey?

No. HighLevel does not convert between the two; duplicate the content and rebuild it in the other builder.
